MetroCare of Springfield is currently accepting applications for HHA’s and PCHM’s for our GAFC (Group Adult Foster Care) Program!
tAdult Foster Care Service Overview
AFC is a service for individuals who need assistance living safely at home. An approval to participate in AFC allows members to receive care which includes up to 24-hour supervision and assistance with daily life activities. An RN and case manager team is assigned to work with the individual and their loved ones to develop a plan of long term care and to provide additional support. Participants of MetroCare of Springfield’s AFC Program will reside comfortably in their home with round-the-clock professional supervision!
Some members of the participant’s family may also qualify to be the direct caregiver! Subject to certain exclusions.
Who Qualifies for AFC (Adult Foster Care)?
Individuals who are at least 16 years of age that need assistance with Activities of Daily Living (ADL’s) such as; Bathing, Dressing, Ambulating (Moving, Walking) Dressing, Eating or Toileting. Participants are able to reside in their home with a caregiver or relocate to an approved caregiver’s home.
