Adult Foster Care (AFC)

Adult Foster Care (AFC) is a specialized service designed to help individuals live safely and comfortably in their homes while receiving the care they need. Through our AFC program, members are provided with up to 24-hour supervision and assistance with daily activities, ensuring their well-being and independence. Each participant is supported by a dedicated team of a Registered Nurse (RN) and a Case Manager, who work closely with the individual and their loved ones to create a personalized long-term care plan and offer ongoing support.

With MetroCare of Springfield’s AFC Program, participants can enjoy the comfort and security of remaining in their own homes while benefiting from round-the-clock professional supervision. In some cases, family members may also qualify to serve as the direct caregiver, subject to specific eligibility criteria.

Who Qualifies for AFC?

Individuals aged 16 and older who require assistance with Activities of Daily Living (ADLs)—such as bathing, dressing, mobility, eating, or toileting—can benefit from our care services. Participants have the option to continue residing in their own home with a caregiver or move to an approved caregiver’s home, ensuring they receive the support they need in a comfortable and familiar environment.

Group Adult Foster Care (GAFC)

Established in 1995, the Group Adult Foster Care (GAFC) Program is specifically designed to support residents of affordable housing by offering the essential services they need to continue living independently in their own homes. This program provides the necessary assistance to help individuals maintain their quality of life and stay within their communities.

What does GAFC do?

The Group Adult Foster Care (GAFC) Program is designed to assist elders and individuals with disabilities in remaining at home and active within their communities. GAFC provides eligible individuals with support for Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Our services include personal care, housekeeping, laundry, grocery shopping, meal preparation, medication reminders, transportation coordination, case management, and comprehensive wellness nursing visits.

Eligibility for GAFC Services:

  • Adults aged 22 and older

  • Individuals who require assistance with one or more of the outlined daily activities

  • Residents of housing that participates in the GAFC program

  • Members of MassHealth or other qualifying insurance plans

Visiting Nurse Association (VNA)

Visiting Nurse Association. VNA services provide in-home healthcare and support to individuals who need medical care but prefer to stay in the comfort of their own homes rather than in a hospital or nursing facility. These services are often provided by a team of healthcare professionals, including registered nurses, physical therapists, occupational therapists, speech therapists, and home health aides.

Key Services Provided by VNA:

  • Skilled Nursing Care: Includes wound care, medication management, monitoring of chronic conditions, and post-surgical care.

  • Therapy Services: Physical, occupational, and speech therapy to help patients regain independence and improve their quality of life.

  • Home Health Aide Services: Assistance with personal care tasks like bathing, dressing, and grooming.

  • Medical Social Services: Support for patients and families dealing with the emotional and social challenges of illness or recovery.

  • Palliative and Hospice Care: Specialized care for individuals with serious or terminal illnesses, focusing on comfort and quality of life.

     VNA services are typically available to individuals of all ages who meet specific medical criteria and are often covered by Medicare,            

    Medicaid, and private insurance.

Preferred Behavioral Health

Preferred Behavioral Health (PBH) is a local mental health clinic dedicated to providing a supportive and inclusive environment that values each person's unique perspective. We offer a wide range of outpatient counseling services, including individual, family, couples, and group therapy. Additionally, we provide therapeutic counseling tailored for workforce employees.

Our experienced counselors work with individuals and groups across all age ranges, ensuring that everyone receives the compassionate and personalized care they need.

Adult Day Health Center (ADHC)

Our centers provide a variety of health-related services and activities to adults who need supervision and assistance during the day, but who do not require 24-hour care. ADHCs are designed to help individuals maintain their independence and remain in their communities while receiving necessary care and socialization opportunities.

Key Services Provided by ADHC:

  • Medical Care and Monitoring: Regular health assessments, medication management, and monitoring of chronic conditions by healthcare professionals such as nurses.

  • Personal Care Services: Assistance with activities of daily living (ADLs), including bathing, grooming, toileting, and feeding.

  • Therapeutic Activities: Structured programs that may include physical therapy, occupational therapy, and speech therapy to help individuals maintain or improve their functional abilities.

  • Social and Recreational Activities: Opportunities for social interaction through activities such as games, arts and crafts, music therapy, and exercise classes.

  • Nutritional Support: Provision of nutritious meals and snacks, often tailored to meet specific dietary needs of the participants.

  • Transportation Services: Transportation to and from the center, as well as to medical appointments, ensuring that participants can easily access the care they need.

  • Care Coordination: Collaboration with other healthcare providers and caregivers to ensure that all aspects of a participant’s care are well-coordinated.

  • Respite Care for Caregivers: Temporary relief for family members or other primary caregivers, allowing them to rest or attend to other responsibilities knowing their loved one is in a safe and supportive environment.

  • Health Education: Programs and workshops that educate participants on managing their health conditions, nutrition, and wellness strategies.

  • Mental Health Services: Counseling and support for participants dealing with mental health issues such as depression, anxiety, or dementia, often provided by licensed therapists or social workers.
